HI pharmacy (also called IV pharmacy or home care pharmacy) is a unique blend of clinical service, patient care, and operational problem solving. Home care pharmacists are part of a multi-disciplinary team that includes nurses, a dietitian, and support staff in areas such as reimbursement, technical, supply, delivery and clerical work. The home infusion pharmacy team leader directs the team in clinical monitoring and distribution services for patients in need of infusion and nutritional therapies. What are the day to day responsibilities of a home infusion pharmacist? The pharmacist is responsible for monitoring patients in need of infusion and nutritional therapy and making sure they receive the appropriate treatment. Patients may receive intravenous antibiotics, parenteral nutrition, chemotherapy, narcotics and inotropic agents (or any combination of the above). The management of these patients requires consideration of many different aspects of patient care including discharge planning, patient education, and the psychosocial needs of the patient. As patients are started on a therapy, it is necessary to evaluate if the patient or caregiver is capable of administering the medication.
